• How to Order Drug & Alcohol Clearinghouse Reports:
  1. Log in to the DOTDriverFiles app.
  2. From the Dashboard, click on “Drivers” in the left navigation bar.
  3. Select the driver for whom you want to order the report.
  4. Click on the box in the top right titled “Drug & Alcohol Clearinghouse”.

5. You will see the following options:
   • Signed Limited Query Consent: The signed consent form is completed during the digital signing of the application and forms. You can review the digitally signed form here.

• Pre-Employment Query: As part of the pre-employment screening, you must run a complete query.
• Annual Query: You must perform a restricted query for each employed driver once yearly after hiring.

6. To submit the request for a pre-employment query:
   • Click the button titled “Submit Pre-Employment Query” and go to FMCSA Clearinghouse.

• Log in to your account or create one if necessary.

• Complete the process on the FMCSA Clearinghouse website.

7. To submit the request for an annual query:
   • Click the button titled “Submit Annual Query” and go to FMCSA Clearinghouse.
   • Log in to your account or create one if necessary.
   • Complete the process on the FMCSA Clearinghouse website.
8. Marking Completed:
   • After completing the process, return to the DOTDriverFiles app and click the second button to the right to mark the query as completed.

• You will be directed to a page to enter the date and relevant information.
• Make sure to check the box that says: “I have a report to be uploaded”. This will refresh the screen and display a file selection button.

• Click the file selection button, select the file, and confirm.
• Finalize the section by clicking the “Upload File(s)” button. This will create a green check mark showing compliance on the main Drug & Alcohol Clearinghouse page.

Note: Users do not need to order a pre-employment and annual query at the same time.
