- Explanation: This section provides step-by-step instructions for drivers on how to complete the employment application sent by a prospective employer through the DOTDriverFiles system. This process ensures all necessary information is provided, supporting documents are attached, and the application is correctly signed for FMCSA compliance.
- How to Complete an Employment Application:
- Receive the Application:
- The prospective employer will send you an employment application via email or text. Click the provided link to start the process.
- Receive the Application:
- Access the Application:
- After clicking on the application link sent by email or text, you will have two options: “Start New Application” or “Return to Application.” Click on the appropriate button to proceed.
- Complete Personal Information:
- Fill out all required personal details, including your full name, address, contact information, Social Security Number (SSN), and date of birth.
- You may be required to provide your driver’s license information, including the license number, state, and expiration date.
- Complete Required Steps:
- On the right side of the screen, you’ll see a list of all steps that must be completed to finalize the application process:
- Application For Employment: Enter your general application information.
- Upload Driver License Copy: Provide a photocopy of your driver’s license.
- Upload Medical Card Copy: Provide a photocopy of your medical card.
- Upload Forfeiture Documents: Provide forfeiture documents, if applicable.
- Violation Record & Annual Review: Complete your violations and annual review agreement.
- Alcohol & Drug Test Statement: Provide your alcohol and drug testing history statement.
- Safety Performance History Investigation: Agree to the previous employer investigation.
- PSP Driver Authorization: Authorize the employer to run a DOT PSP report.
- Alcohol & Drug Testing Policy: Review and agree to the employer’s alcohol and drug testing policy.
- General Work Policy: Review and agree to the employer’s general work policy.
- On the right side of the screen, you’ll see a list of all steps that must be completed to finalize the application process:
- Experience Section:
- In the Experience section, you must select “Yes” or “No” for all equipment classes.
- If you select “No,” you do not need to enter additional details.
- If you select “Yes,” you must complete all three boxes to the right, including start and end dates and miles driven with that type of equipment.
- Driver’s License Section:
- You must complete all required fields related to your driver’s license, including the license number, issuing state, and expiration date.
- Additionally, check the box next to every endorsement you possess to ensure the information is accurate and complete.
- Accidents/Crashes Previous 3 Years:
- You must enter all incidents from the past three years. This includes the date, location, nature of the accident, and any injuries or fatalities.
- The employer will electronically verify all incidents with the appropriate reporting authorities.
- Moving Traffic Violations Previous 3 Years:
- Enter all moving traffic violations from the past three years. Include details such as the date, location, and type of violation.
- The employer will electronically verify all violations with the appropriate authorities.
- Forfeitures Previous 3 Years:
- You must answer all questions related to forfeitures within the past three years.
- If you select “Yes” for any forfeiture, you must provide an explanation in the space provided.
- Work History:
- You must provide your most recent three years of work history. Enter details such as employer names, positions held, and dates of employment.
- If additional entries are needed, click the “Add Another Previous Employer” button to ensure all three years are fully accounted for.
- Fair Credit Reporting Act & Disclaimer:
- Review the Fair Credit Reporting Act disclosure and the disclaimer at the end of the application.
- Click in the signature box, and a pop-up window will appear asking for your cell phone number.
- Enter your cell phone number, and you will receive a 6-digit code via SMS.
- Enter the code to digitally sign the application.
- Click “Continue” to proceed to the next page.
- Upload Driver’s License and Medical Card:
- On the next page, you will be prompted to upload images of the front and back of your driver’s license. Click “Choose File” to select the files from your device and then click “Upload File(s).”
- After uploading your driver’s license, you will be asked to upload a digital copy of your Medical Clearance Card using the same process.
- Violation Record:
- The system will then prompt you to review your “Record of Violations.” If you missed any violations earlier, you can add them now.
- You can also tick the box that says “I have no violations.”
- Click in the signature box at the bottom to digitally sign this section.
- Pre-Employment Employee Alcohol & Drug Test Statement:
- You will be asked to answer the following questions:
- Have you tested positive or refused to test on any pre-employment drug or alcohol test administered by an employer during the past two years?
- Have you tested positive or refused to test on any pre-employment drug or alcohol test administered by an employer for safety-sensitive transportation work covered by DOT agency rules during the past two years?
- Select “Yes” or “No” for each question.
- Follow the same procedure to digitally sign and click “Continue.”
- You will be asked to answer the following questions:
- Safety Performance History Investigation & PSP Driver Disclosure:
- Review the “Safety Performance History Investigation” and “PSP Driver Disclosure & Authorization” sections.
- Digitally sign both sections following the same steps as before.
- General Consent for Limited Queries of the FMCSA Drug and Alcohol Clearinghouse:
- Review the explanation and consent for limited queries of the FMCSA Drug and Alcohol Clearinghouse.
- Digitally sign as before.
- Review and Sign Employer’s Policies:
- Review the employer’s “Alcohol & Drug Testing Policy” and “General Work Policy.”
- Digitally sign both policies following the same procedure as before.
- Final Submission:
- After completing all required steps, the application process will be complete.
