  5. In the Qualification Checklist, find the section titled “Annual Record of Violations & Review of Driving Record (Preceding 12 Months)” and click “Complete Now”.

6. A page will load where you can review the current status of the driver’s record and violations.
7. Click “Request Violations” to automatically email the driver for their input if not completed already on the application.

8. The driver receives an email, clicks the hyperlink, fills in all violations, and clicks on the signature at the bottom.

9. A window pops up to verify the cell number and sends a 6-digit code. The driver enters the code and clicks “Sign Document”, which digitally signs the document.
