- How to Complete the Safety Performance History Investigation:
- Log in to the DOTDriverFiles app.
- From the Dashboard, click on “Drivers” in the left navigation bar.
- Select the driver whose Safety Performance History Investigation needs to be completed.
- Click on the “Qualification Checklist” tab in the top navigation bar.
- In the Qualification Checklist, find the section titled “Safety Performance History Investigation” and click “Complete Now”.
- Click the button titled “Review & Sign Document” to open the Safety Performance History Investigation document.
- The hiring company will need to electronically sign the document. Click on the signature box, and then use your cell phone to confirm and digitally sign. After signing, click “Submit” to proceed.
- The driver is responsible for completing their work history on the electronic application. The hiring company will verify this work history by email by clicking the button “Send Email to Previous Employer” on each workplace listed.
- The hiring company must attempt to verify the previous work history three times within 30 days. If the previous employer does not respond, the hiring company remains compliant, and the system will show a time and date-stamped email attempt to confirm the previous history.
- For successful verifications, you can click “View Investigation Document” to see the history from that company.
- Previous employers without a response will show the dates of attempted verification or a note that says, “This employer has been marked as non-applicable,” as appropriate. You can also upload a digital copy of the work history verification if available.
