• How to Verify Medical Examiner’s National Registration:
  1. Log in to the DOTDriverFiles app.
  2. From the Dashboard, click on “Drivers” in the left navigation bar.
  3. Select the driver whose Medical Examiner’s Certificate you need to verify.
  4. Click on the “Qualification Checklist” tab in the top navigation bar.
  5. Click on “Medical Examiner Verified on National Registry of Certified Medical Examiners” and click the “Complete Now” button.

6. There are two buttons: “Download Blank Form” and “Upload Completed Form”. Download the form.

7. Print and complete the fields on the document and sign it.
8. Scan the completed document.
9. Upload the completed document by clicking on the “Upload Completed Form” button and selecting the completed form document.
10. This will then create a green checkmark on the Qualification Checklist.
